Scottish Clinical Coding Standards

Clinical and health related data are collected on various national patient based datasets. To ensure data is recorded consistently and accurately, nationally agreed coding systems are used. The coding systems used are the International Statistical Classification of Diseases and Health Related Problems, Tenth Revision (ICD-10) for diagnoses and the Office of Population Censuses and Surveys, Fourth Revision (OPCS-4) for procedures.

To support of these nationally agreed coding systems, ISD have provided clinical coding staff with Scottish Clinical Coding Standards (previously known as Coding Guidelines) since May 1996.

An index of the articlesDownload PDF file [472 Kb] published in Scottish Clinical Coding Standards and a flowchart Download PDF file [24kb] explaining the process of resolving coding queries is also available. The index contains the title of standards both current and obsolete but does not contain detail about the standard itself.

Clarification of recording of co-morbidities on SMR data - new guidelines.

Please note that documents entitled 'consolidation' are compilations of all current standards at any given time.
